2023 SouthEast LinuxFest - Post mortems (also known as Root Cause Analysis amongst other names) are difficult for most organizations to do effectively. Often times even in companies who encourage open collaboration sharing and learning end up conducting these sessions in such a way that blame is ultimately assigned. This leads to staff learning to hold back or defend their positions which in the end leads to a post mortem that is incomplete at best and down right fallacious at worst.
