Patient died after delayed heart readings due to 'break down in communication'

Mr Waterhouse received a call about someone who had been experiencing chest pain for 18 hours. They knew he needed to be sent to the specialist heart unit at Harefield Hospital but received no response so took the patient to Watford General Hospital.
