Labour: Ockenden report findings shows 'institutional' failings

Shadow minister for patient safety Feryal Clark said of the Shrewsbury and Telford Hospital NHS Trust's failings: 'This was institutional failure of a system which failed to take up the many opportunities to realise that it had a serious problem.'
