Hospital trust 'failed to learn and improve' according to report

A report into the UK's biggest maternity scandal at Shrewsbury and Telford Hospital has found that 201 babies and 9 mothers could have survived. The chair of the report Donna Ockenden said the trust 'failed to investigate, to learn and to improve.'
